Hours & Location

Remote Sessions are available most weekdays between 10am - 4pm. Earlier or later appointments are possible.

In-Person Sessions: designated Thursdays, Fridays and Saturdays at Evergeen Natural Health Center, 1033 Basin Avenue in Bismarck, ND.

In-Person Sessions: designated Wednesdays in New Town.

Forms for appointments can be downloaded below. The Intake Form is needed for your first appointment. Choose from either Microsoft Word, which you can fill out digitally and e-mail, or print and bring to your appointment; or PDF, which you can print, fill out, and bring to your appointment:
